Please use this online Proof of Claim Form.  The Assignee is aware that many Licensors may not know the total sum owed by GoDigital, Inc./Distribber.  The Assignee intends to complete an accounting and notify Licensors prior to the February 28, 2020 deadline of the calculated amount of unpaid royalties or license fees.

 

If you do know the total sum due to you, please feel free to submit the form now.  There is not need to send screen shots of your current Distribber dashboard as the Assignee has access to them.  If you have specific documentation to support errors in the amount shown on your dashboard, that would be helpful.
